How to apply for health insurance card reissuance due to damage or loss online?

To minimize the risk of Covid-19 transmission and save time for citizens, the health insurance card reissuance due to damage or loss has now been integrated into the national public service portal. Follow this article to learn how to proceed!

 Step 1: Access the National Public Service Portal at https://dichvucong.gov.vn/

Step 2: Register an account on the National Public Service Portal.

This step is for those who do not have an account.

Choose “Citizen” as the registration method and select the Social Insurance logo to register an account.

Fill in the required information (using the social insurance information being managed).

Enter the “Verification Code” and click “Register”.

Step 3: Log in with the account you just registered on the National Public Service Portal.

Log in by entering the information: ID card/ Citizen ID card; Password and Verification Code.

Step 4: Proceed with the procedure for health insurance card reissuance due to damage or loss.

After logging in, select “Information and Services”, then “Online Public Services”.

Next, select “Health insurance card reissuance due to damage or loss”. Choose “List of public services”.

Select “Submit online” in the “ Health insurance card reissuance due to damage or loss” section.

The screen will switch to the Social Insurance’s online public service portal.

Then enter the “Social insurance number” and “Check code”, click “Search”, and enter the remaining information to complete.

Citizens can choose the address to receive the dossier at the “Dossier reception and result return section” or receive it “Via postal service”.

Finally, enter the “Check code” and select “Confirm”. The Social insurance agency will send a confirmation notification and an appointment date for the result to the registered phone number.

If citizens choose to receive the result via postal service, the health insurance card will be sent directly to the registered home address. For option at the Dossier reception and result return section, one shall visit the Social insurance agency to receive the health insurance card.

On the National Public Service Portal, citizens may apply for reissuance, change, or adjustment of information on the social insurance book, health insurance card (Except for requests to aggregate working periods before 1995)…
